CDR-credentialed practitioners who have completed a CDR CPEU Prior Approved activity that they feel does not meet CDR’s Content Criteria or CDR CPEU Prior Approval Program policies may complete this form to communicate complaints to CDR. Once this form is submitted, CDR staff will review the submission and determine next steps. Review outcomes are shared with the CPE Provider only.
